The Contract Holder:                          

• Has read the attached DHARMA Gym for All (DGFA) RULES, POLICIES, SAFETY RULES, OFFICERS, COACHING STAFF and FEES and accept the rules and conditions of membership to the club as specified.


DHARMA GYM FOR ALL adheres to all British Gymnastics Policies – British Gymnastics is our Trampoline governing body. All policies can be viewed on www.british-gymnastics.org.

All DHARMA GYM FOR ALL coaches are qualified, hold Safeguarding and DBS qualifications. All Coaches and Volunteers have GDPR training & safeguarding.

 DHARMA GYM FOR ALL (DGFA) has qualified Welfare Officers.

• NB – This contract is binding until such time that a resignation letter is received giving two months written notice.


• Has accepted the training times offered and all associated fees.

           

• Has accepted that to resign from DGFA or reduce session numbers, two months written notice is required otherwise will be liable to pay fees. (Transfers from other clubs, have a 6 week probation period. Decision made after 6 weeks. Either party can terminate within or by the end of 6 weeks.)                                                                                    


• Has agreed to give children permission (under 18 years of age) to take part in stated activities at local or away venues with DGFA and provide permission for the application of emergency medical care as it arises.


• Is aware there is a copy of the BG Code of Practice available to be read on BG website.


• Understands that although the Club will endeavour to minimise any risk of injury, accidents may still happen.


• It is a requirement that the coach is informed that the participant is physically fit and healthy to participate, particularly after illness or injury.


• Is aware and agrees that applications to film or photograph the participants during training or away from the Club must be scrutinised and approved by a minimum of two authorised and responsible adult coaches.


• Is aware that the Club uses filming/videoing as a useful coaching aid and agree to videoing for this purpose. A minimum of two club-designated persons will always be present during any filming. Care will be taken to securely store the video materials to avoid misuse. (Permission will be sought from parents and participants for videoing for any other purposes).


• Has agreed/gives permission for their child (under 18 years of age) to have their photograph or videoed image to be published in DGFA literature, website and the press.


• Is aware and agrees that apparatus should be assembled or erected and dismantled to the manufacturer’s instructions and the handling of apparatus should be supervised at all times by a qualified coach.  All persons handling equipment must receive adequate instruction on storage and the above. I agree to request advice/instruction from a qualified coach before initially handling any equipment.


• Is aware and agrees/gives permission for their child if under 8 years of age to participate in British Gymnastics proficiency scheme and/or club/local competition.


• Agrees that DGFA will only act in ‘Loco Parentis’ during children’s session times and when in the training hall.


• Agrees & gives permission for their child’s (under 18 years of age) personal details to be online administration system.


• Gives permission for their child (under 18 years of age) to be transported, by car or coach, if necessary, through prior arrangement with parents/guardians.


• Participants who may be susceptible to Atlantoaxial Instability need to supply a written statement from a medical practitioner that the individual may participate in trampolining
